Water comes in through the side of the housing marked "IN", then flows through the cartridge, where filtration takes place. After the water has undergone the filtration process, clean water flows out through the side marked "OUT".

The blue top is for a smaller nipple and the aqua top is larger!! I returned and got the aqua top because this one didn’t fit!
