I'm 155lbs and usually get Large for my bottoms and underwear. These were tight. Considering the bloating that occurs post surgery for some situations, theses didn't work for me. Having them on would have made me more uncomfortable.

These definitely run small and I'd recommend sizing up. I also feel that they could be less snug around the thigh, they remind me of a mens cut in the sense. Otherwise the sides hold well and they are super easy off and on. Helpful post surgery.
