To my horror, this specialty plate came in 4 parts -- the face plate, plus the 3 loose, individual port squares (which fit like puzzle pieces IF you can get them to snap in fully and correctly!). This whole set is cheaply constructed. There's cheap "Made in China" plastic, and then there's cheeeeeeap plastic. This is the latter. To assemble, you either have to attach the coaxial cable heads to the loose ports, and then snap each into the face plate holes, OR you have to snap the loose ports into the square holes, and then screw on the coaxial cable heads. Either way, it's a nightmare. If you do the first method, it takes about a hundred attempts to get the port squares to snap in fully and completely. If you try the second method, there's not sufficient space between the coax ports and the plastic tabs to screw on the metal coax heads. Bottom line: this wall plate is poorly designed, manufactured at the cost of pennies, and comes in pieces that the poor buyer must fiddle with for an hour before he/she can attach it to a wall. I had to buy this because I had no other choice, but I highly recommend against it.
