This simple form can be used for many purposes. It is a basic contact form, very straightforward and easy to transcribe to digital format if desired. A good value also for the price! Do recommend!

They do the job but yes they're huge (6 inches!) 100% my fault for not reading, though it's nice people don't have to write tiny, thus making their entry form illegible.

The size I'd the form is good, everything isn't so close together how some forms are. These will be perfect for what what need.

The forms are ni e and generic... just really over-priced. But if your in bind and need something quick, these will do the trick if you don’t mind the expense. I usually look for fair market value. These could be priced a lot better.

These do the job. There's a place for all pertinent info but they are huge! I guess it's my own fault for not reading how big they were. It is listed in the description. I've never seen entry forms so big, I never thought I needed to check the size. If size doesn't matter to you, this is a great price and they'll do the job!

These are good sized. They are basic enough for my purpose...I use them to hold drawings for a free prize with an insurance quote.
