We have purchased UB-04 forms from this company many times (10 times or more) and this time we found a typo within the form that we hadn't noticed in previous orders (but we did pull up old claims that had been printed months ago and confirmed they had the same issue- we just never noticed). Box 45, Serv Date , their form says Serv Cate. The bigger issue that we experienced with the most recent batch was that the forms were not cut properly, the first 50 or so claims were cut too high and therefore threw the alignment of the forms off making them unusable. You can see on the bottom of the form where the cut was supposed to be.
