Loved 'em.We are putting the original signatures in a binder and trashing the last copy----too hard to read the copy. We gave up having patients sign/print and them do as they wish. Great form....should have had it sooner.

Great sign in sheets. A bit pricey, but they are perfect for my office. Customer service is great and I would definitely purchase from them again.
