Does the 4K60 HDMI inserter combine input from DVI-only Macs and toslink (or L/R analog)?
There is a switch that selects what type of audio you are going to send down the HDMI cable. They do not combine, you have to choose which one you are using.

This supports 4k video with stereo inserted audio. BUT, make sure you plug it into something that provides enough power. I didn't realize the brick I plugged into was only a 10w supply and it couldn't do 4k on that. Read the instructions and make sure you do it right.I had some issues with the mono price inserter because even after getting it replaced it only supported single channel audio, this solves that problem, very happy with it. I will update it if I have any issues

I'm normally hesitant to buy this sort small-brand Chinese hardware that floods Amazon, but there were no well known brands offering this part so I gave it a try. Happily I have had no issues. The option to use optical, 3.5mm, or hdmi pass through is very nice.I have not noticed any timing or latency with the audio matching up with the video.I took the enclosure off mine because I will be putting it inside a different system. the board is compact and clean.

My computer graphics card does not output an audio signal on the HDMI output, so I had to find some way to combine audio and HDMI video. This did the trick!

The HDMI Audio Extractor is a great device. It worked perfectly for me. I used it to separate the audio from my PS4 and connect it to my surround sound system without any problems. It was really easy to use and set up. It comes with an HDMI to Optical Audio Adapter & 3.5 Audio Out Jack. I was able to enjoy high-quality audio on my surround sound system. The 3.5mm L/R Stereo Audio option was my choice for connection, and it provided crisp audio to the surround sound.

I typically don't review Amazon products but I got wonderful support from the company when my original purchase became defective about 6 months after we started using it. Once I described the issues I had, they promptly sent a replacement which worked and solved the problem. I have had such poor customer support from so many companies that those who stand behind their products are worthy of giving a shout out. I should also note that at no time did they ask me to give a review of them or their product!

My fancy gaming laptop just can't output HDMI audio when connecting to a TV via HDMI. This device solves the problem; good thing I also had an extra HDMI cable, an extra 3.5 mm audio cable, an extra micro USB cable, and a free USB port for power. Now I can play all the fancy games my fancy gaming laptop allows for on the biggest display in my room.
