I didn't notice when I purchased but these are just a smidgen to short. When you fold the CMS 1500 claim form so that the address shows but nothing else, then the flap doesn't close over it properly and I have to use tape to hold it down. VERY inconvenient!!

It works, but it is small for the claim’s form. You need to adjust very well to see the address complete in the window, it make it harder to close the envelope after you blend the form. Just 1/8in bigger will be perfect. Not sure yet if I will return them.

Poorly packaged and handled. The box was crushed when I received it. Many of the envelopes were damaged, very few would lay flat anymore, so I cannot run them through our printer. The adhesive is not sticky, they do not stay sealed unless you tape them. Overall, this was a waste of money, not even worth shipping them back.

This is supposed to be for HCFA 1500 claims but in order for the address to show you have to fold through the zip code which is fine, but then the claim still doesn't fit in the envelope nicely. I guess I shouldn't have tried to save money and should have ordered from Quill because the envelopes for clams are bigger and actually fit when folded properly. I would NOT buy again.
